We are seeking a dedicated Registered Nurse to join our Home Health team. The ideal candidate will provide compassionate and high-quality care to patients in the comfort of their homes. We cover Lake and parts of Porter County Indiana.

Responsibilities:

Conduct patient assessments and develop individualized care plans
Administer medications and treatments as prescribed by the physician
Monitor and record patient progress and vital signs
Educate patients and their families on proper healthcare strategies
Collaborate with physicians and other healthcare professionals to ensure optimal patient outcomes

Experience:

Minimum of 1 year of experience in nursing, preferably in home health.
Proficiency in basic life support (BLS) techniques
Ability to perform essential nursing skills such as administering medications, conducting assessments, and providing patient education.
Familiarity with alimentary procedures and patient care protocols

Requirements:

• Indiana professional license
• CPR certification
• Current Physical and TB test
• Home Health Software and SOC preferred
• Valid driver's license and auto insurance

SOC rate: $90.00
